Herzen Moscow Oncology Research Institute – branch of the National Medical Research Radiological Centre. Patients with multiple myeloma were administered 1 capsule of NEPA 1 hour before conditioning, patients with lymphoma – 2 capsules: 1 hour before conditioning and 72 hours after the first dose of the drug. The primary endpoint was the complete response rate defined as no emesis and no rescue medication during the acute and delayed phases.</p><p><bold>Results.</bold> Complete response in the overall cohort was achieved in 81 % of patients both in the acute and delayed phases. In the high-dose melphalan group and LEAM-conditioning group (lomustine, etoposide, cytarabine, melphalan), the rates were also identical in the acute and delayed phases and were 75 and 88.9 %, respectively. Complete control in the entire cohort was achieved in 66.7 % during acute phase and 76.2 % during delayed phase. Corresponding rates in high-dose melphalan group were 66.7 and 75 %, respectively, and in LEAM-conditioning group – 66.7 and 77.8 %, respectively.</p><p><bold>Conclusion.</bold> NEPA monotherapy demonstrates high efficacy in auto-HCT recipients receiving highly-emetogenic pretransplant conditioning, while simplifying the antiemetic prophylaxis protocol as compared to four-drug-based regimens.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Комбинация ингибиторов серотониновых 5-HT3-рецепторов и NK1-ингибиторов с дексаметазоном и оланзапином является основой профилактики тошноты и рвоты, индуцированных высокоэметогенной химиотерапией, в том числе используемой перед трансплантацией аутологичных гемопоэтических стволовых клеток крови (ауто-ТГСК).</p><p><bold>Цель исследования</bold> – оценить эффективность и токсичность комбинированного препарата нетупитант/палоносетрон (NEPA) в монорежиме в рамках профилактики тошноты и рвоты, индуцированных предтрансплантационным химиокондиционированием.</p><p><bold>Материалы и методы.</bold> В проспективное исследование включены пациенты (n = 21), которым была выполнена ауто-ТГСК в условиях МНИОИ им. П.А. Герцена – филиала НМИЦ радиологии. Больные множественной миеломой принимали 1 капсулу NEPA за 1 ч до начала кондиционирования, а пациенты с лимфомами – 2 капсулы: за 1 ч до начала кондиционирования и через 72 ч после приема 1-й дозы препарата. Первичной конечной точкой исследования являлась частота достижения полного ответа (отсутствие рвоты и неназначение метоклопрамида) в острую и отсроченную фазы.</p><p><bold>Результаты.</bold> Частота достижения полного ответа в общей когорте в острую и отсроченную фазы была идентичной и составила 81 %. В группах пациентов, получивших кондиционирование высокодозным мелфаланом (MEL200) и кондиционирование в режиме LEAM (ломустин, этопозид, цитарабин, мелфалан), эти показатели в острую и отсроченную фазы были также идентичны и составили 75 и 88,9 % в первой и второй группах соответственно. Частота достижения полного контроля в острую и отсроченную фазы в общей группе составила 66,7 и 76,2 % соответственно.
