The presence of bone plasmacytoma with soft tissue components and spinal cord compression, pathological compression fractures of vertebral bodies worsen prognosis by increasing the probability of neurologic deficit development and quick progression at any stage. Therefore, the use of multidisciplinary approach is justified.A clinical observation of a 65-year-old patient with multiple myeloma manifesting through bone plasmacytomas with massive soft-tissue components and complicated by pathological compression fracture of the Th<sub>2</sub> vertebra with neurologic deficit is presented. After the appearance of clinical symptoms of compression, surgical intervention was urgently performed (decompression laminectomy, subtotal resection of the tumor at the Th<sub>2</sub>–Th<sub>3</sub> vertebra level) with partial mobilization of the patient in the early postoperative period. Antitumor drug therapy was started (daratumumab, lenalidomide, dexamethasone – D-Rd regimen) demonstrating positive effect.</p></abstract><trans-abstract xml:lang="ru"><p>Использование высокоэффективных комбинаций лекарственных агентов в 1-й линии терапии множественной миеломы способствует увеличению выживаемости во всех подгруппах пациентов, в том числе высокого риска. Наличие костных плазмоцитом с мягкотканными компонентами и компрессией спинного мозга, возникновение компрессионных патологических переломов тел позвонков ухудшают прогноз, повышая вероятность появления и быстрого нарастания неврологического дефицита на любом этапе. В связи с этим применение мультидисциплинарного подхода является обоснованным.В статье представлено клиническое наблюдение пациента 65 лет с множественной миеломой, проявляющейся костными плазмоцитомами с массивными мягкотканными компонентами и осложненной компрессионным патологическим переломом Th<sub>2</sub>-позвонка с развитием неврологического дефицита. В кратчайшие сроки от момента появления клинических симптомов компрессии было выполнено оперативное вмешательство (декомпрессивная ламинэктомия, субтотальное удаление опухоли на уровне Th<sub>2</sub>–Th<sub>3</sub>-позвонков) с частичной активизацией пациента в раннем послеоперационном периоде.
