Pirogov National Medical and Surgical Center) and 61 of the second group – in regional low volume hospital (Centrosoyuz Hospital). The endpoints of the study were the short-term outcomes: surgery duration, blood loss, postoperative hospital stay, intra- and postoperative complications, lymph node harvested and the quality of specimen.</p><p><bold>Results. </bold>Surgery duration as well as blood loss in two groups were similar (258 min vs 240 min, p = 0.47 and 100 ml and 100 ml, p = 0.89, accordingly). Hospital stay after surgery was significantly less in the first group (9 days vs 16 days, p &lt;0.01). The frequency of postoperative complications of type I–II according to the Clavien–Dindo classification was higher in the first group (6/8 vs 1/8, p &lt;0.05) but reoperations performed more in the second group due to III Clavien–Dindo complications (2/8 vs 7/8, p = 0.012). Grade 2–3 rectal cancer specimen quality was 94.7 % vs 77.8 %, p = 0.13 and harvested lymph nodes &gt;12 was 71.9 % vs 78.7 %, p = 0.485 in first and second group, respectively.</p><p><bold>Conclusion.</bold> Hospital volume does not affect rectal cancer specimen quality, surgery duration and blood loss, if the surgical interventions are performed by one experienced surgeon but associated with the incidence of postoperative complications and the length of stay.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – оценить влияние уровня медицинского учреждения на результаты хирургического лечения  пациентов с колоректальным раком.</p><p><bold>Материалы и методы. </bold>В ретроспективное исследование включены 93 пациента с колоректальным раком, оперированные одним хирургом в 2 медицинских центрах г. Москвы с 2018 по 2020 г. В 1-ю группу включены пациенты, оперированные в крупном федеральном медицинском учреждении – ФГБУ «Национальный медико-хирургический Центр им. Н.И. Пирогова» Минздрава России (32 пациента). Во 2-ю группу вошли пациенты, оперированные в медицинском учреждении общего профиля с небольшим коечным фондом и минимальной оснащенностью стационара – Больнице Центросоюза Российской Федерации (61 пациент). Конечными точками исследования являлись  непосредственные результаты: время операции, объем кровопотери, продолжительность стационарного лечения, интра- и послеоперационные осложнения, количество выявленных  лимфатических узлов и качество удаляемого макропрепарата.</p><p><bold>Результаты. </bold>Продолжительность хирургического вмешательства в 1-й и 2-й группах достоверно не различалась (258 мин против 240 мин, р =0,47). Объем кровопотери в обеих группах составил 100 мл, р = 0,89.  Продолжительность послеоперационного койко-дня была достоверно меньше в 1-й группе (9 дней против 16 дней, р &lt;0,01). Частота послеоперационных осложнений I–II типа по классификации Clavien–Dindo была выше в 1-й группе (6/8 против 1/8, р &lt;0,05), однако повторные операции по поводу осложнений достоверно чаще выполнялись во 2-й группе (2/8 против 7/8, р = 0,012). Качественная мезоректумэктомия (Grade 2–3) выполнена в 94,7 и 77,8 % в 1-й и 2-й группах соответственно, р = 0,13; более 12 лимфатических узлов выявлено соответственно у 71,9 и 78,7 % пациентов, р = 0,485.</p><p><bold>Заключение. </bold>Уровень медицинского учреждения не влияет на качество хирургического лечения больных колоректальным раком, если хирургические вмешательства выполняются одним хирургом,  прошедшим кривую обучения и имеющим большой опыт выполнения таких операций.
