Selection of treatment program for patients with newly diagnosed or recurrent FL is based primarily on the size of the tumor. While use of rituximab-containing regimens led to increased progression-free survival, in some cases tumors are refractory to rituximab. Understanding of molecular mechanisms of FL pathogenesis and action of anti-CD20 monoclonal antibodies allowed to develop new drugs with several advantages compared to rituximab. According to the data of large randomized trials, use of glycoengineered monoclonal anti-CD20 type II antibody obinutuzumab in combination with chemotherapy in patients with FL has manageable toxicity profile and longer progression-free survival and event-free survival compared to use of rituximab.</p></abstract><trans-abstract xml:lang="ru"><p>Фолликулярная лимфома (ФЛ), являясь наиболее распространенным типом индолентных лимфом, характеризуется рецидивирующим течением и гетерогенностью клинических проявлений. Выбор программы терапии пациентов как с впервые выявленной ФЛ, так и с рецидивами определяется преимущественно объемом опухоли. Наряду с тем, что использование ритуксимабсодержащих режимов привело к увеличению выживаемости без прогрессирования, в некоторых случаях отмечается рефрактерность опухоли к ритуксимабу. Понимание механизмов молекулярного патогенеза ФЛ и действия анти-CD20 моноклональных антител способствовало разработке новых препаратов, обладающих рядом преимуществ по сравнению с ритуксимабом. По данным проведенных крупных рандомизированных исследований, применение гликоинженерного моноклонального анти-CD20-антитела II типа обинутузумаба в комбинации с химиотерапией у пациентов с ФЛ наряду с управляемым профилем токсичности привело к более длительной выживаемости без прогрессирования и бессобытийной выживаемости по сравнению с группой больных, получавших ритуксимаб.</p></trans-abstract><kwd-group xml:lang="en"><kwd>follicular lymphoma</kwd><kwd>obinutuzumab</kwd><kwd>antitumor effect</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>фолликулярная лимфома</kwd><kwd>обинутузумаб</kwd><kwd>противоопухолевый эффект</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Kridel R., Sehn L.H., Gascoyne R.D.
